I (Customer name Above), hereby authorize Real Beauty USA, Inc. to debit the bank account provided for agreed-upon purchases as documented in applicable invoices or purchase orders. I understand that my banking information will be securely stored on file for future transactions on my account with Real Beauty USA, Inc. In the event of an ACH transaction being rejected for Non-Sufficient Funds (NSF), submission error, or other bank-related return reasons, I understand and agree that Real Beauty USA, Inc. may, at its discretion, resubmit the ACH debit transaction within fourteen (14) days. I also understand and agree that a return item charge may be assessed for each returned ACH debit transaction. I certify that I am an authorized user of this bank account and will not dispute the scheduled transactions with my bank, provided the transactions correspond to the terms indicated in this authorization form. I understand that this authorization will remain in effect until I provide written notice of cancellation to Real Beauty USA, Inc., allowing a minimum of 15 business days for processing.
